Our members are invited to fill out the Champions Family Information and Involvement Sheet. This sheet is a five-page (8.5 X 11) document that can be printed, filled out, and turned in to the church office. - Child Safety Check Form (PDF)
Champions seeks to provide a safe and healthy learning environment for all of our children. All Champions members who wish to work with children aged 18 or younger MUST fill out and turn in a Child Safety Check Form.
